Doc takes a liking to some of his grandson's friends. He sees little Izuku often enough, the tyke gets sick fairly often, and even more often once the boy gets diagnosed as “Quirkless” (which he is, now. It's easy enough to doctor x-rays, although he does feel a modicum of guilt for shattering his dreams like that). Young Katsuki comes from time to time, sometimes to treat burns he cause by his own quirk, sometimes to accompany his best friend as they do a myriad of “tests” to see if Midoriya is really quirkless.
“Deku,” Katsuki declares in a voice full with all the righteousness a 4 year old can, “has a quirk, you're just a hack.”
(Ah, so Izuku must have used his ability before then, probably on Katsuki himself. Force Activation is such an interesting quirk and he's glad he has given it to AFO already. He can use it better)
But being called a hack and the myriad of other names Katsuki could think of (this boy is most definitely Mitsuki'a child) while Izuku tries to calm the fiery blonde down (and oh, how familiar is that to Mitsuki and Inko in their youth and teenager years, if only AFO had perfected this science, two decades, no, even a decade ago and maybe he could have had the two for himself instead).
As AFO's doctor he has access to such marvelous inventions and science that one can almost call it /magic/ and it is one of these, a way to hypnotize & manipulate growing minds that he plans to use today. It's done wonders on the Shimura boy, so why not play with it a bit as well, see if it can be just as long lasting but much more subtle?
It starts like any other of Izuku's checkups, the little boy comes in with Inko and Katsuki and with a bit of coaxing, the woman leaves so he can talk to the boys alone.
The act of hypnotizing them is simple enough, and Doc takes so much pleasure at seeing their eyes glaze over, become empty and mindless and just waiting for him to fill them with commands.
Doc starts slowly, that first day just an introduction to the pleasures they will give him, instructing them on how to jack him off and swallow his cum off of each other's faces. With his quirk allowing him to bring people to him, Doc merely has to confirm that his pets are alone and no one will miss them for a few hours (easier with Izuku, what with a single mother. Katsuki he normally has to schedule, as he often has friends to entertain) and he – and AFO – get the chance to play.
Just like before he documents the way he uses the boys. It's always good to have the possibility of scandal if either of them find their way in heroics after all.
Doc has plenty of both. There's only two possibilities, and while AFO could have put something in his pets, the fact that All Might is now teaching at U.A. and seems to have a close mentorship with his pet makes that a more likely scenario.
It's a pity that Tsubasa failed at bringing Izuku to him to test and see if he actually had (Doc doesn't mourn his grandson's death at all, other than for the loss of data and work and the chance to have at least one of his pets at his side)
Months later, Doc gets instructions and contingency plans and one very specific order – to bring Katsuki to AFO.
When Shigaraki and his Vanguard Squad steals him away from the heroes, Doc makes his way to AFO's side, hidden. The moment he sees All Might at the bar, Doc uses his quirk to pull the boy to his and AFO's location.
Doc is so conflicted – Katsuki is right there, and all it would take is a certain phrase and the boy would become pliant and obedient to both him and AFO, a crushing blow to the Symbol of Peace. But no, he has orders and he's told to wait in the sidelines, even as AM appears, even as he finds out Izuku and his classmates are there to rescue Katsuki (it would be so easy to pull them back go him with his quirk). But Doc let's them go, leaves so he, like many others, can watch AM deal another devastating blow to the man he most respects.
Doc can see Katsuki and Izuku stand, shoulder to shoulder m, as All Might tells the world that he is still there, sees them surrounded by concern friends, and has to wonder if 15 and 16 is young enough that the machine will still work, will let him bring new bodies under his control. (How he would love to dissect the Todoroki boy, see how to create a perfect combination of quirks so cleanly like Endeavor had. To pull apart one of the Iida's engines to replicate en masse. He's seen reports of the Monoma boy, how would a copy quirk synergize with Izuku's original? With AFO itself? And a boy with brainwashing, the possibilities!).
But for now, Doc will wait for the perfect time to show All Might that he has already lost when his “Wonder Duo” has already been owned by villains long before he's met them.
